At work we wanted it to be employees-only. So we set up our own server (synapse + element) in our internal network and disabled any kind of federation. So you have to use our VPN to be able to connect to it. This kind of setup would obviously be the wrong choice for an open source project.
But Matrix is very flexible and works well in multiple kinds of setup structures. For example the government of France uses Matrix with federation over several internal servers as basis for their secure chat platform. A good example for a successful setup with federation over public servers would be the FOSDEM 2021 online conference. You could seamlessly take part in the discussions from any matrix server connected to the public federation at matrix.org and it was also bridged to IRC.
